hye uols...this is my last entry for today..hahaha...berape dah erk entry ari ni je..mood ngah ade..so this is news about my dad..encik bolang bin majid...pernah suffer colon cancer 3 years ago..now he is much more better..thank you ALLAH..

TO: visiting oncologist HTAA s/t DR MARRY, ONCOLOGY MO HKL.

History.

-low anterior rescution was done in 2007. He was referred to oncology department, HKL, for chemotherapy and radiotherapy in 2007.he only completed 1 cycle of mayo’s regime and defaulted subsequent follow up or any therapy given to patient. On april 2009, patient came back to hoshas for treatment. He was started on mayo’s regime and referred to oncology, HTAA. Upon review by oncology,HTAA patient was started on folfox regime on 2009 (19/11/09) and completed 6 cycles on 18/2/2010.

Diagnose

-CT abdomen and pelvis 25/2/2010. A known case of low rectal ca post operative and chemo status with multiple small hypodense lesions in the liver. These hypodense lesions could represent liver cyst or cystic metastasis. Lung bases are clear. No bony metastasis.

-CT 9/8/2010. Rectal ca post surgery with abdominal and inguinal lymphadenopathy.

-Colonoscope 8/12/2010. No recurrence. No anastamotic structure.

-currently, patient stable and tolerating orally.

Purpose:

-we plan for closure of colostomy after onco review.Kindly, r/v this patient and offer your expert mx. TQ
